Click here for 'Request for Services' form in downloadable PDF format.This is the basic form we MUST have completed BEFORE we can provide any counseling services - including the new Ask Yuba SBDC online  (it is not required to attend our workshops and seminars).  So, how can you get this completed form to us as quickly and easily as possible? 

Download the form, print it, complete and sign it, and do any of the following:

  • Fax it to our Yuba City office at (530) 822-0163.  That will give you instant access to our counselors, or
  • Mail it to our Yuba City office at 1227 Bridge Street, Ste. C, Yuba City, CA 95991.  That will give you access to our counselors as soon as it arrives (usually 1-2 days), or
  • Drop it off at our Yuba City office, during normal business hours, at 1227 Bridge Street, Ste. C, Yuba City, CA 95991.  That will give you access to our counselors as soon as you bring it in, or
  • Bring it with you when you meet with any of our counselors, at any location, the first time.

We are working on a fully-interactive form you can complete online - so check back often.
